The IRS Form 1095-B is a federal tax document that shows qualifying healthcare coverage you and members of your household received in the previous year. The IRS no longer requires individuals to file Form 1095-B with their federal income tax return. As a result, GlobalHealth will not automatically provide the IRS Form 1095-B to its members.

Your IRS Form 1095-B will be mailed to you within 30 days of receiving your request. If you have any questions about your IRS Form 1095-B, please call Customer Care at 1-877-280-5600 (TTY: 711).
